The ContinuLink system is a web-based proprietary software solution accessed securely via the Internet, used in Home Healthcare, Private Duty, Hospice agencies and Per-Diem Supplemental Staffing, as the core operating system for these businesses. ContinuLink handles all of the operational flows from patient/client, referral sources, payer, caregiver and employee intake, to scheduling, billing, payroll, collections as well as clinical functions including patient assessments, medication profiles, care plans, clinical documentation and record keeping. The ContinuLink platform includes components such as a Point of Care system for use by clinicians, a web portal for physicians to access the relevant patient care documents to sign and certifiy, a web portal for clients and family to monitor their care, and a web portal for workers to access their schedules and see patient and agency related information.
